How Does It Work?

XEOMIN® works at the neuromuscular junction, the microscopic communication point where nerve meets muscle. Every time you frown, squint, or raise your eyebrows, motor neurons release a neurotransmitter called acetylcholine that triggers the target muscle to contract. This release depends on a protein called SNAP-25 (Synaptosomal-Associated Protein of 25 kDa), which is essential for neurotransmitter vesicles to fuse with the cell membrane and release their contents.

IncobotulinumtoxinA binds to receptors on cholinergic nerve terminals, enters the nerve cell through receptor-mediated endocytosis, and once internalized, selectively cleaves SNAP-25 11. Without functional SNAP-25, acetylcholine cannot be released, and the muscle relaxes. The result is not immobility. It is a precise, targeted quieting of overactive muscles. The lines they create soften, while your natural range of expression stays intact.

This chemodenervation is inherently temporary. Over 3-4 months, axonal sprouting and regeneration of functional SNAP-25 gradually restore full neuromuscular transmission 11. Most clients describe the sensation as a brief pinch, well-tolerated and over quickly.

For comprehensive face-and-neck treatment, your provider addresses each area systematically: frown lines (20 units across 5 injection sites), forehead (20 units across 5 sites, treated simultaneously with glabellar lines), crow’s feet (24 units across 6 sites), and platysmal bands (typically 30-50 units with conservative dosing) 5, 10, 11. The maximum recommended dose for simultaneous upper facial line treatment is 64 units 11.

FDA-Approved vs. Off-Label Treatment Areas

XEOMIN® is FDA-approved for glabellar lines (frown lines), and as of July 2024, for simultaneous treatment of forehead lines and crow’s feet alongside glabellar lines 12. Neck treatment for platysmal bands, bunny lines, lip flip, masseter reduction, and other areas are off-label but commonly performed applications. Keep your head elevated for 4 hours after treatment. Gently exercise treated muscles for 1-2 hours to help distribution. Avoid touching or massaging treated areas and skip strenuous exercise for 24 hours, though light walking is fine. Avoid heat exposure (saunas, hot showers, steam rooms) for 48-72 hours.
