What Is XEOMIN®?

How Does It Work?

XEOMIN® works at the neuromuscular junction, the microscopic communication point where nerve meets muscle. Every time you frown, squint, or raise your eyebrows, motor neurons release a neurotransmitter called acetylcholine that triggers the target muscle to contract. This release depends on a protein called SNAP-25 (Synaptosomal-Associated Protein of 25 kDa), which is essential for neurotransmitter vesicles to fuse with the cell membrane and release their contents.

IncobotulinumtoxinA binds to receptors on cholinergic nerve terminals, enters the nerve cell through receptor-mediated endocytosis, and once internalized, selectively cleaves SNAP-25 11. Without functional SNAP-25, acetylcholine cannot be released, and the muscle relaxes. The result is not immobility. It is a precise, targeted quieting of overactive muscles. The lines they create soften, while your natural range of expression stays intact.

This chemodenervation is inherently temporary. Over 3-4 months, axonal sprouting and regeneration of functional SNAP-25 gradually restore full neuromuscular transmission 11. This built-in biological reversibility is a fundamental safety feature, meaning every treatment’s effects are always self-resolving.

The Purified Formulation Advantage

The complexing proteins present in other botulinum toxin type A formulations, particularly hemagglutinin HA-1, have been identified as immunostimulatory factors that may contribute to neutralizing antibody formation over time 8. XTRACT Technology™ separates the active 150 kDa neurotoxin from those hemagglutinin and non-hemagglutinin proteins, so what ends up in the vial is the neurotoxin without the accessory protein load 11.

In a long-term study of 91 patients treated for neurological conditions, none of the 59 who had only ever received incobotulinumtoxinA tested positive for neutralizing antibodies, while 2 of the 32 who had previously been on other formulations did 7. Across XEOMIN®‘s full clinical program of 2,649 patients, 0.3% tested positive for neutralizing antibodies after treatment — and not one of them lost treatment response as a result 11. In the glabellar lines trials specifically, no patient developed neutralizing antibodies at all 11.

Results typically last 3-4 months, providing a predictable treatment rhythm that fits into your life. Individual results vary based on muscle strength, metabolism, and treatment area.

Effective Neck Rejuvenation

For platysmal bands, those vertical cords that become more prominent with age, the published evidence is encouraging and honestly quite small. In an open-label pilot of 25 women who each received 60 units divided across four bands, every participant met the responder threshold — at least a one-point improvement on a five-point band scale — at Day 14, and 96% still met it at three months 5. Onset can be rapid: in a single-arm study of 23 patients treated with 15 units per band, 65.2% showed visible improvement by Day 3 and all of them by Day 8 6. Neither study had a placebo group, and neither was large. Neck treatment for platysmal bands is an off-label but commonly performed application, and your provider will talk through what that evidence does and does not promise before you decide.

The Opening (Days 1-7)

The first visible reduction in muscle movement typically appears between days 3 and 5, when XEOMIN®‘s SNAP-25 cleavage begins blocking neurotransmitter release at enough neuromuscular junctions to produce visible smoothing. The prescribing information puts the median onset of effect at two to seven days after injection 11. For neck treatment, improvement can appear at the early end of that window — 65.2% of the 23 subjects in a single-arm study showed visible change by Day 3 6. Building Momentum (Weeks 2-4)

The maximum recommended dose for simultaneous upper facial line treatment is 64 units 11. For platysmal bands — an off-label area — published guidance recommends a conservative 30 to 50 units in a single session 10.

FDA-Approved vs. Off-Label Treatment Areas

XEOMIN® is FDA-approved for glabellar lines (frown lines), and as of July 2024, for simultaneous treatment of forehead lines and crow’s feet alongside glabellar lines 12. Post-Treatment Care

Keep your head elevated for 4 hours after treatment. Gently exercise treated muscles for 1-2 hours to help distribution. Avoid touching or massaging treated areas and skip strenuous exercise for 24 hours, though light walking is fine. Avoid heat exposure (saunas, hot showers, steam rooms) for 48-72 hours.
